Impact Aid Surveys
Greetings Falcon Families!
It is time for the annual Federal Impact Aid Survey given each year to all students. Please review the attached letter, complete the attached document, and return to Mr. Padgett via email (directions below). Please complete and return by Monday, December 6th, 2021 if possible. If you have any questions, please contact Mr. Padgett (his email is listed below). If you are unable to follow the directions below, please feel free to print, fill out, and return your survey to the front office of the school.
· Open "Impact Aid Survey Flora" attachment
· Complete the form and sign (printed name on form is fine)
· Save it with the changes
· Send the completed document as an attachment to Mr. Padgett at firstname.lastname@example.org no later than Monday, December 6th.
